This job description is intended to help individuals and organizations to recruit people into the PF position. It has been developed specifically to support Primary Care Networks in their capacity to support member practices advance the Patient’s Medical Home.

How was it developed?

  • The job description was developed following the AMA Human Resources’ processes and tools. The AMA’s job description template and position development guidelines were informed from Chartered Professionals in Human Resources practices.
  • The content for the (i) purpose and scope, and (ii) duties and responsibilities or work performed sections are aligned with the PF Competencies Framework.

How to use it?

  • PCNs can customize this resource to fit their existing HR templates.
  • This resource may also be used to support the understanding of the PF role for Primary Care transformation in Alberta.
