Sample email content
(Insert Clinic Name) is changing, so our team can improve the way we provide the care you need, in your community, when you need it. By using more of a team approach with our Family Physicians, Physician Assistant, Registered Nurse and Medical Office Assistants through the Blended Capitation Model, we will be able to:
- See you when you need an appointment with your doctor or one of our team members who works closely with them
- Provide you with more thorough care
To ensure we can continue to provide you with the best care possible, we need you to sign a form agreeing that you consider (Insert Clinic Name) to be your Medical Home, the first place you go for routine medical help, where your doctor and health team is. We provide this information to Alberta Health so that they know who our patients are. By signing the agreement, you agree to:
- Contact our clinic first whenever you have a non-emergency health need
- Update the clinic with changes in your health or personal information
This will allow your doctor and your team to have a full understanding of your needs. We want to be your first stop for all your healthcare needs and continue to have a relationship with you where we work together to meet your goals and achieve the best health possible.
The attached form (Ensure form is attached to the email) will be available for you to sign at your next visit. If you would like, you may print and complete the form ahead of time to bring with you to your next appointment.
