The facility number identifies the physical location where the service was provided. Each EMR stores the facility ID for each provider for billing purposes. Facility ID numbers are needed when filling out CII/CPAR paperwork.
Click your EMR for instructions on how to find your Facility ID number.
The facility ID is available in the billing module. One place it may be viewed is in Claims > Daily Claims or Unsubmitted Claims > Options > Set Location (here you will see the locations or facilities available).
To find the facility ID number, click on your username on the top right of the Ava window, then click Clinic Management. This will bring you to the Clinic Information page, which displays the Facility Number.
To find the facility ID number, open the Settings window from the toolbar running down the left side of the CHR main page. From there, scroll down to the CII/CPAR tab to find the Facility Number.
You can find the Facility Number in Healthquest by opening the AHC Claim Editing window.
You can find the Facility Number in Med Access from the Update Facility page.
To find the facility ID in PS Suite, open the Bill Book for any physician that works at the clinic and bills in PS Suite. The easiest way to do this is to ask a physician to show you their bill book and record the Facility ID.
The facility ID can be found on the billing screen.
