How to Apply for the PCCM

Step 1 | Review PCCM Resources
The AMA has developed a range of support tools, information sessions and resources to assist with transitioning to the PCCM. These resources will help you determine if the PCCM is right for you, offer guidance on the enrollment process, changes to billing, panel management and more.

Step 2 | Meet Pre-Requisites
To be eligible for the PCCM, physicians must
- participate in CII/CPAR
- have a minimum panel size of 500
- work a minimum of 400 hours over 40 weeks per year
PCCM panel calculations can be viewed on the AMA Member Dashboard (member login required). In some cases, a physician's PCCM panel size will differ from their CPAR panel size, or the panel size calculation associated with the Panel Management Support Program (PMSP).

Alberta Health provides data updates to the AMA on a monthly basis. If your panel size has recently increased to exceed 500 and your data has not yet been updated, we encourage you to check back later.

Step 3 | Express Interest
Physicians interested in the PCCM can express their interest on their AMA Member Dashboard (member login required) – only qualifying physicians will have access to the Expression of Interest form.
The PCCM Expression of Interest is open to physicians who meet the eligibility criteria, as determined by Alberta Health data. If the EOI form is not visible on your dashboard, it indicates that you do not currently meet the prerequisites.

Step 4 | Obtain PCCM Application Form
In March 2025, eligible physicians who have expressed their interest will receive a Primary Care Physician Compensation Model Clinical Alternative Relationship Plan (cARP) Application Form via email from the AMA. This form is unique to you and cannot be shared with other physicians.

Step 5 | Obtain PCCM BA
To obtain a PCCM Business Arrangement Number (PCCM BA), physicians will need to fill out the Primary Care Physician Compensation Model Clinical Alternative Relationship Plan (cARP) Application Form and return it to [email protected].

Step 6 | Billing with your PCCM BA
Physicians who receive a PCCM BA can begin to bill services under PCCM as of April 1, 2025.
After April 1, 2025, physician applications for the PCCM will be processed on an ongoing basis with implementation for approved providers beginning the first business day of the following month. Therefore, applicants can begin billing services under the PCCM on the first day of the month following the issuance of their PCCM BA.
In addition to holding a PCCM BA, physicians must continue to maintain their FFS BA to claim services outside of the PCCM (e.g., for care provided to patients without an Alberta Healthcare card, specialty focus areas and services provided in hospital).
