Spend three days together connecting, learning and talking about the future of primary care in Alberta and the innovative possibilities. Forum will feature a welcome reception on night one with a fun and entertaining opening keynote speaker. The main two days feature engaging plenary and keynote speakers, a panel discussion and a variety of breakout sessions.
This year we’re exploring the theme The Path Ahead and will focus on navigating the monumental changes to primary care in Alberta and moving ahead as a united group.
Learning Objectives
- Transmit provincial direction in primary care and to advance the strategies and structures that enable primary care and health for all those who reside in Alberta.
- Inspire the audience to embrace the future and innovation so they can build and/or mature the Patient’s Medical Home model, team-based care, safe Transitions of Care, Integration of Care, PCN/Regional Networks, and other system enablers.
- Provide opportunities for attendees to question and interpret examples of how to apply change management and achieve changes at scale.
- Provide attendees space and time to decide what they will do differently as a group, as an individual, and as a leader to promote successful change and wellness. Create a plan with next steps and commitments.
- Provide attendees options for tangible next steps, guidance and direction: equip them to execute.
About Forum
The Primary Care Strategic Forum (formerly “PCN Strategic Forum”) is an annual multi-day conference that focuses on primary care transformation in Alberta. This is where primary health care system leaders and multi-disciplinary providers will gather to learn about the important initiatives underway that advance the Patient’s Medical Home in an Integrated Health Neighborhood. The event alternates between Calgary and Edmonton.
