Care continuity tool reaches critical adoption

86% of Primary Care Network physicians who carry a panel are now using the Community Information Integration & Central Patient Attachment Registry (CII/CPAR)!


It was developed by Alberta Health back in 2016 after local physicians identified the need for it. Since then, the use of the tool has grown from just a few test clinics in 2018 to being the new norm, adopted by more than 850 clinics across Alberta. Millions of Albertans now benefit from increased continuity of care as CII/CPAR allows patient information to flow securely between clinic EMRs and Alberta Netcare. This makes our province a national leader in health system integration. 

The progress in the past four years is nothing short of astounding.

This important achievement in care continuity improvement comes after more than six years of collective effort by physician leaders, PCN leadership and staff, eHealth/Alberta Health, AHS, Alberta Medical Association (AMA), AMA-Accelerating Change Transformation Team, EMR vendors, participating physicians and their teams, and others. Congratulations to all those that helped Alberta realize this incredible achievement. 

The remarkable adoption rate exceeds the long-standing project target of 80% participation that was set several years ago. It also puts thousands of physicians in a prime position to consider adopting the Physician Comprehensive Care Model (PCCM) as CII/CPAR adoption is a requirement to participate.
