The team-based care model harnesses the skill and expertise across the health care delivery team to meet patients’ needs efficiently and effectively. This tandem approach has many benefits for patients and the team caring for them, including:
Over the last year, a wide range of supports have been provided to teams across the province to build capacity and help them work together in support of patient care needs.
The AMA-ACTT also provides guides, training and videos to support team-based care, including past webinars, Learn@AMA training and tools. More than 300 users took advantage of the on-demand content in the past year.

The Networks of Practice members offerings are just one part of the data-informed team-based care materials, instruction and advice available -- all built with AMA-ACTT's nearly decade-long track record of innovating in the space.
