The forms you may need:

Core Plan

• Core Plan Application
• Core Plan Benefit Change Form
• Core Plan Claim Form (Alberta Blue Cross)
• Participation Agreement

Cost-Plus Plan

• Cost-Plus Plan Benefit Change Form
• Cost-Plus Plan online claim form (For members only. Claims for physician's/clinic employees must be submitted using the Cost-Plus Plan Claim Form.)

Below are the forms you may need:

• Submit your CME claim online
• Application for Benefits 
• Log into the Member Dashboard to complete the Payment Request form.
• Eligibility Status form 
• Access your statement (members, log in to access your CME statement)

This form determines eligibility for the Continuing Medical Education, Medical Liability Reimbursement and Parental Leave Programs.

• Application for Insurance form (AMA disability, life, critical illness, professional overhead expense)
• Application for Accidental Death and Dismemberment Insurance form
• Beneficiary Designation forms:
  • Life Insurance (AMA policy #4328)
  • Accidental Death & Dismemberment (AMA policy # 50092201)

• Log into the Member Dashboard to complete the Payment Request form.
• Eligibility Status form

Claim the benefit:

• Each eligible physician parent must complete an application form
• You must submit your application within 18 weeks of the delivery or adoption placement date.

To update banking details for AMA membership and/or negotiated benefits, log into the Member Dashboard to complete and return the payment request form with a void cheque.
