Primary Care Alliance (PCA)

The Alberta Primary Care Alliance (PCA) brings together the experience and expertise of physician partners who are involved in the advancement and improvement of primary care provided to patients, families and communities in Alberta.

Purpose

Building on existing member organization strategic directions, the Alliance’s goal is to foster a coordinated and aligned approach to meet the needs of patients, caregivers, families, primary care teams, PCNs and the healthcare system today and into the future.

The Primary Care 2030 White Paper describes a health system with an orientation towards primary care and community-based care that will generate better patient outcomes; better and more equitable access; and lower costs (in high-performing health systems, primary care accounts for one third to one half of total health expenditure).

Primary Care Alliance and Specialty Care Alliance (SCA) – Improving Transitions of Care

The 2017 Auditor General’s Report, Better Healthcare for Albertans, identified integration and continuity of care as a key to improving quality and managing costs. At the AMA’s 2018 fall Representative Forum, delegates were shown the short film, Falling Through the Cracks: Greg’s Story. In May 2018, the PCA and SCA developed a collaborative working group to focus on improving integration between primary and specialty care. Our hope is to have a scalable, flexible, functional strategy that will better integrate the transitions between primary and specialty care in order to prevent patients from falling through the cracks.
